What the I-693 exam covers

The USCIS Form I-693 is required for adjustment of status (green card) applicants. Only a USCIS-designated Civil Surgeon can complete and sign it.

Arrive between 8am and 7pm, any day. Bring your ID and vaccination records. Our staff will get you started right away.

Medical examination by our Civil Surgeon

Our USCIS-designated physician conducts the full I-693 examination — medical history, physical exam, required lab tests, and vaccination review.

Missing vaccines administered

If your records are incomplete, we administer required vaccinations on-site during the same visit — no need to come back.

Sealed I-693 envelope — ready to submit

Once complete, your Form I-693 is sealed in the required USCIS envelope. Submit it with your green card application or bring it to your USCIS interview.

Who needs a Form I-693 immigration physical?

Yes. The sealed envelope is given to you. You submit it directly to USCIS with your application or bring it to your interview. Do not open the sealed envelope — it must remain sealed for USCIS to accept it.
