Arrive between 8am and 7pm, any day. Bring your ID and vaccination records. Our staff will get you started right away.
Our USCIS-designated physician conducts the full I-693 examination — medical history, physical exam, required lab tests, and vaccination review.
If your records are incomplete, we administer required vaccinations on-site during the same visit — no need to come back.
Once complete, your Form I-693 is sealed in the required USCIS envelope. Submit it with your green card application or bring it to your USCIS interview.
Anyone applying to adjust their status to lawful permanent resident (green card) within the U.S. must submit Form I-693. It may also be required for certain other immigration benefits. Your immigration attorney can confirm your specific requirements.

Bring whatever records you have. If records are unavailable for certain vaccines, we can either administer the missing vaccines or in some cases use blood titers to confirm immunity. We'll guide you through the process on the day of your visit.
Yes. The sealed envelope is given to you. You submit it directly to USCIS with your application or bring it to your interview. Do not open the sealed envelope — it must remain sealed for USCIS to accept it.
